• If you wish to cancel cards (and not order replacement cards) please complete this form.
• Online cancellations must be submitted by an authorised person (signature not required).   To view a copy of our Conditions of Use click HERE.
• Faxed cancellations must be signed by an authorised person.
• The card(s) you are cancelling MUST be destroyed.
• If you wish to cancel more than 20 cards (for the same account) please provide details in the comments box (on company letterhead if faxed).
• If you wish to cancel cards for other accounts please submit a new form for each account.
